Health and Well Being for Elderly Projects  

Health and well being for elderly (HWBE) Project North Bihar.
Bihar is the regular destination of flood and flood is annual guest of Bihar.18th August 2008, the Kosi River breached its embankment and displaced 3.065 million residents from 1,704 villages in North Bihar. This national calamity engulfed hundreds of thousands acres of land , crops, homes, sweeping away people, cattle and leaving behind a trail of death and destruction .  
The Kosi-flood in 2008 has brought unprecedented disaster and devastation for the people of 14 districts of Bihar. Of these districts Supaul is the first and worst affected district.  Help Age India did relief work in Supaul district. The support to the tune of approximately 200,000 EURO. Furtherance to this , HI started early recovery work in  58 wards of 20 villages where it earlier did  relief work.HWBE project started from June ‘2009- December’2012 (42 months) with the support of CORD AID (Catholic Organization for Relief and Development Aid).total amount of support is 472448 EURO.
The overarching goal is to make a resilient community towards disaster and to improve the quality of life of older persons and reduce the burden of their health, social and economical problems. The five objectives help in achieving this goal. These objectives are as follows.

  • To mobilise communities through participative methods and enable them to access healthcare and social protection services.

The main notion of this objective is to put in place mechanisms to ensure availability, accessibility and affordability of a decent life and good health among the aged. Following is the activity performed to measure the achievement of progress.

Activity 1: Provide access to primary and emergency medical care.

Followings are the indicators against which one can measure the change.
Target 1. - 5,600 families in 20 villages have access to primary medi-care through Mobile Medical Unit.
To carry out the activities staffs have been oriented about project, a base line survey has been done. Beneficiaries have been selected painstakingly against the vulnerability criteria enshrine in the proposal.

  • Achievement-
  • 2532 elders of 20 villages have been registered and accessing regular medi- care through the MMU. Among these 2532 elders 1258 are male and 1274 are female.
  • 1111 elderly have access to the Pathology service through MMU. Among these 1111 elders 548 are male and 563 are female.
  • 02 cataract surgery camps organized for with support of Indian Red Cross Society and District Blindness Control Society, Supaul and 120 elders successfully operated.
  • 975 elders screened for Anaemia and 364 elders screened for Hypertension and diabetes through Special camp organized 04 days in a month
  •  A special cardiac camp for 92 identified case of hypertensive and diabetic elders to cardiac problem.

Target 2. - Training of 20 CHVs on community health and epidemic management

  • Achievement- through 3 training organised in Oct ’09, Aug’10 and Feb’11, 20 CHV have been trained over community health & epidemic management during emergency/flood.

Target 3. - Health kits are available with 20 community health volunteers (CHVs) with essential preventative medicines and water purification tablets in 20 villages by month 2
Achievement- health kits are available with 20 CHVs with 3 days (22-24th Sept 09) at Bihar Voluntary Health Association in Patna. Apart from that, Medicine related for Anemia, Hypertension and Diabetes also available in Health Kit to make emergency supply for avoiding discontinuation of regular medication of patients suffering of these diseases.
Target4. - Palliative Care Training is provided to 116 volunteers (two from each ward) by end of month18.
Achievement. – 47 village volunteers and community health volunteers provided palliative care training in 02 batches with support from Palliative care Project, Cuddalore.

Target 5. - Physio-therapy Unit to be launched in each block by end of 24 months.
Achievement- 04 stationary physiotherapy unit established in 04 villages of project area, i.e. Bishanpur Shivram and Nirmali of Basantpur block, Balua in Chhatapur block and Govindpur in Pratapganj block of Supaul district. 802 (404 men and 398 women) elders are registered and 3269 times accessed to get service from physiotherapy centre. Apart from that, CHV and caregivers/relative provided physiotherapy training.

Target 6. - Each of the 20 villages will have a volunteer identified to mitigate disasters
Achievement. – 20 community health volunteers working in 20 villages.

Target 7. - 25 locally relevant medical practitioners are identified and will form first level health defense in 20 villages by the end of the project.

Achievement-

  • 4 days training for 25 practitioners of alternative medicine providing medical services in epidemic management from 12th to 15th May’10.
  • 02 Health Education workshops organized at Govindpur inaugurated and addressed by Civil Surgeon, Supaul, qualified doctors, panchayat leader and at Balua addressed by Assistant Engineer of Public Health Engineering Department, Supaul to create awareness on Health and Hygiene, Sanitation, Daily diets and various government schemes available under National Rural Health Mission and Gram Swasthya and Swachchhta Samiti. Apart from this, cleanliness drive as well as awareness through pamphlet, banners, role plays and folk songs organized in all villages of project area.  
  • To decrease the socio-economic vulnerability of older persons through their organisation into viable and sustainable Elder Self-help Groups (ESHGs) and to facilitate their federation at local, district and national levels. 

To achieve this objective following activity is to be performed.
Activity 2: Formation and strengthening of Community Based Organizations towards improved preparedness and resilience among the communities at risk

 Following is the milestone to achieve the activity
Target .1- 80 Elder Self Help Groups (ESHGs) are formed with participation of 850 elders or more by month 4.

Achievement-

  • 37 women ESHG (557 members) and 40 men ESHG (592 members) formed having saving Rs.36120/- and total saving of Rs. 442347/- at end of February 2011
  • 25 more groups are pipelined and total number will cross 100 by end of March/April 2011

Target2. - 20 ESHG village level federations are achieved and registered by month 7.

Achievement – 18 villages federation formed and registration of under process. The ESHG of 02 villages joined federation of neighbor village due to less number of ESHGs (02 each) for greater strength and support.

Target 3. - 1 district level federation of 80 ESHG is realized by month 9.

Achievement- Incomplete

Target 4. - 20 DPMCs established, legally recognized by month 8 and maintain emergency rations from start-up support.
Achievement - Formation of 20 DPMC in all villages completed and registration are in progress. Emergency rations is in process

  • To provide home – and community – based care and support to older persons who are physically, socially, psychologically, or otherwise, marginalised and challenged

To realize this objective following is the activity to be performed.

Activity 3: Promotion of Sustainable livelihood activities

Target 1. -Grading of ESHGs on National Norms.

Achievement - Grading and provision for seed capital to all formed groups are in progress and will be completed by April / May 2011

Target 2. - Start-up capital is available to each group for interloaning.

Achievement - All 77 groups are functional and having total membership contribution of Rs. 407419 and total saving with interest of Rs. 442347/- only.
Monthly saving of all groups are Rs. 36120/- only (as on 28.02.2011). Opening of bank account is in process

Target 3. - At least 50% of the ESHGs have successful bank-linkages by month 36.

Achievement – In process

  • To facilitate and provide health and social services to older persons

The activity under this objective is

Local disaster management components, constituting stocks of emergency and relief items 

Are available for ffunctional and immediate rescue and relief at village / community level at the

Time of disaster

Following is the target to achieve the activity

Target 1. - 20 DPMCs established maintain emergency rations from start-up support.

Achievement- Formation of 20 DPMC in all villages completed and registration are in progress.

Target 2. - 20 DPMCs are trained in CBDRR approaches

Achievement- A training for DPMC for DRR analysis and action plan is planned in April 2011

Target 3. - 20 grain and dry fodder banks are formed by month 12

Achievement- In process

Target 4. - 150 families in each village have one week’s dry food rations in preparation for the next flood

Achievement- ………………………………………..

Target 5. - 200 life jackets are provided across the 20 villages for rapid rescue

Achievement- 120 no of Life Jackets and 70 no of megaphone available with the community

Target 6. - Evacuation and rescue maps are available at each ward (58 wards)

Achievement- In process. Training is planned in next week and will be ready by May 2011. Social Map is ready

  • To reduce the physical vulnerability of older persons in the event of disasters and to improve their ability and access to relief at such times

To reach this objective the activity is as follow.

Activity 5: Elders and the other marginalized voice demands for their entitlements.

Targets are as follow to ascertain the activity.
Target 1.
- All ESHG and DPMC members are made aware of the relief codes

Achievement- A training for DPMC for DRR analysis and action plan is planned in April 2011 and training of CVs for understanding of various government schemes is proposed in April and June 2011 and ESHGs for relief code

Target2. - 2 CSO sensitization workshops are conducted

Achievement- A sensitization workshop is also proposed in April ‘11 with support of Dalit Watch
Target3. - Advocacy and communication material, in vernacular is available across all the wards on entitlements of elders and the relief code.

Achievement- as advocacy materials and will be completed by end of April ‘11
Apart from these proposed activity in objective 5; a cycle rally from State Capital to project area were conducted by the volunteers to raise issue for rights and entitlements of elders.
A literacy camp is proposed in month of April 2011 with support of PRAXIS and Government functionaries.
A video documentary on ESHG is in progress.
